Why most men start losing their hair
Losing you hair can be very frustrating. We all know that.
But why do men lose their hair…?
Hair loss in men is more commonly known as ‘androgenic thinning’ or male pattern hair loss.
This type of hair loss can be more or less severe. The extent of hair loss can be devided into several stages as shown below:

Most men start losing their hair in the temples and the crown.
The reason why this happens is that the hair in these areas are exposed to a sex hormone called dihydrotestosterone (DHT), with powerful androgenic actions.
When the hair follicles are influenced by DHT, they start to miniaturize.
Miniaturization of hair follicles means that:
- The hairs get smaller and thinner
- The growth phase of the hairs is shortened
- Hair shedding happens faster
- It takes longer time from hair shedding to a new hair starts to grow out
This process is illustrated in the figure below…
The result is hair thinning and can lead to complete male pattern baldness.

Figure 2: The male sex hormone, DHT, results in miniaturization of the hair follicles. In other areas of the scalp such as the back of the head and the sides, the hair follicles are unaffected by DHT. That’s why the hair is preserved in these areas throughout life.
How old you’re when you start to experience androgenic thinning as well as the extent of hair loss is mainly determined by genetics.
Thus, men who are genetically deficient in Type 2 5-alpha-reductase do not develop androgenic hair loss.
Moreover, Type 2 5-alpha-reductase has been detected in scalp hair follicles, and balding scalps contain increased Type 2 5-alpha-reductase activity and DHT levels.
Taken together, these findings provide a rationale for the use of Type 2 5-alpha-reductase inhibitors in the treatment of men with androgenic hair loss.
Now, let’s talk more about finasteride… and the treatment of androgenic baldness.
What is Finasteride?
Finasteride (better known as ‘Propecia’) is one of the few proven medications on the market which is used to treat male pattern hair loss.
Finasteride is a specific and potent inhibitor of Type 2 5-alpha-reductase, which decreases the formation of DHT from testosterone.
What type of hair loss can be treated by Finasteride?
There are many types of hair loss with different symptoms and causes. The most common one, no doubt, is androgenic thinning.
Finasteride works excellent for treating androgenic hair loss in men aged 20-50 years who suffers from:
- Mild to moderate hair loss
In men with extensive hair loss (i.e. Norwood V-VII), hair loss is usually better treated by a hair transplantation and post-operative oral finasteride to stop further hair loss after surgery.
Diffuse thinning in young men, however, can also be treated by Finasteride with excellent results.
How does Finasteride work?
As stated earlier, androgenic thinning is caused by too high concentrations of the male sex hormone, DHT.
DHT is made from testosterone, and the production of DHT is regulated by Type 2 5-alfa-reductase.
Finasteride works by inhibiting the expression of Type 2 5-alfa-reductase.
Hence, Finasteride lowers DHT levels in the scalp, and reduces DHT’s harmful effect on hair follicles.
This is shown in the figure below.

The mechanisms of finasteride are:
- Lower the concentration of DHT in the scalp
- Restore the hormonal balance around the hair follicles
- Increase the follicle growth phase

The recommended dosage of oral Finasteride is 1-1.25 mg daily.
