The hair phrases in men with and without hair loss

All hairs – even the hairs on our scalp – begin as vellus hairs. The vellus hairs are the very small, fine hairs which can be seen in the forehead if you look very closely.

Eventually vellus hairs morph into terminal hairs and are marked by a darker, thicker, and more noticeable appearance.

As terminal hairs grow, they go through three main phases. They include anagen, catagen, and telogen.

  • Growth phase (anagen phrase) In this phrase the hairs on the scalp is growing. The growth phrase can last anywhere from 2 – 8 years. In men, the growth phrase is typically much shorter than in women.
  • Transition phase (catagen phase) – In this phase, the hair follicle cease growing and is the intermediate between growth and rest. This phase only last 2-3 weeks.
  • Rest phase (telogen phase) – This phase last 3-4 months. During this phase, hair shedding increases, and old hairs fall out. After the rest phase, a new growth phase takes place and new hairs take the old hairs place. Hence, the hair cycle restarts.

The three phases are shown in the figure below:

Thereafter, the hair follicle starts to shrink and gets smaller until – in the end of the telogen phase –hair shedding occurs. Thereafter, a new growth phase begins, and the cycle starts all over.

An average Caucasian male have between 100,000-150,000 hairs on the scalp.

This varies with natural hair color.

Hence, blondes have an average of 150,000 hairs, the average redhead has 90,000, and those with black or brown hair have an average of 110,000 to 100,000 hairs.

In a person without hair loss up to 90 % of all hairs on the scalp are growing. The remaining 10 % of the hairs are either in transition or the telogen phase.

It is normale to loss an estimate of 75-100 hairs every day, while the same number of hairs start in a new growth phase.

In other words, there is a balance…. so people who don’t suffer from hair loss either gains or losses more or less hairs than they already have.

But what about people with hair loss…?

…In men and women who suffer from hair loss, more hairs shed than the number of hairs starting a new growth phase.

As an example, a person suffering from hair loss might loss an average of 120 hairs per day while only 100 new hairs start growing. Hence, it this example there is an imbalance between the number of hairs that are created and the hairs that are lost.

The consequence over time is thinning….

When a person has lost approximately 50 % of all hair in an area of the scalp, the hair loss starts to get obvious to others due to lack of coverage.

Side effects of hair thinning?

We all know the symptoms of hair loss…! The hair gets thinner…

But what are the side effects…? Anxiety, lower self-esteem, frustration? Well, it’s not funny at all to start losing your hair. Nicely looking hair has immense importance for most men and women, and it does matter when the hair starts to shed.

With age, hair loss affects an increasing number of men. In most men hair thinning begins in the early to late 20’s.

Hence, hair loss can occur twenty per cent in your 20s, 30 per cent in your 30s and so on… By the age of 50, about 85 per cent of men have significantly thinner hair.

Likewise, female pattern hair loss affects up to 1 in 3 women during a lifetime. In women, the hair loss is typically more diffuse…

Where to expect hair thinning?

Male pattern hair loss typically begins around the temples and often starts with thinning rather than total hair loss.

As the receding hairline progresses, the hair in the middle may stay closer to the forehead. This V-shaped hair growth in front is often called a “widow’s peak”.

In others, the hairline moves back across the top of the head. This often leaves a ring of hair around the top of a bare scalp.

In areas of hair thinning, you’ll notice at close examination, that the hairs are thinner and shorter and some of the hairs might eventually get very small and fine.

This process is called miniaturization and describes the regression of terminal hairs into vellus hair…

What is the most common form of hair loss in men?

There are many different types of hair loss. A complete review of all diseases which can give hair loss is beyond the scope of this introduction.

Instead, we’ll focus on ‘androgenic thinning’ (also known as androgenic alopecia) – the main reason for hair loss in both men and women.

Searching the World Wide Web (Google or Bing), you might also meet the terms ‘male pattern hair loss (MPHL)’ …

A number of mechanisms seems to play a role in androgenic hair loss. when that’s said, we understand the mechanisms much better in males than in women.

Why some men lose their hair while others don’t

So, now let’s take a closer look on why some men experience male pattern baldness while others keep there hair throughout life.

The results of the negative influence from DHT are:

  • The hairs get smaller and thinner
  • The growth phase of the hairs is shortened
  • Hair shedding happens faster
  • Longer time from hair shedding to a new hair starts to grow out

 

This process is illustrated in the figure below. The terminal hairs are miniaturized via the negative influence of DHT. The result is hair thinning and can lead to complete male pattern baldness.

This negative influence can effectively be stopped by oral Finasteride as well as topicals including anti-androgens and vasodilators (such as Minoxidil).

What's my risk of going bald?

Genetics plays an immense role in the development of male pattern hair loss.

Meanwhile, research shows that more than 200 genes are responsible for the development and extent of hair loss (source).

Hence, predicting hair loss via genetic testing is difficult.

This also means, that just because your father is bald in his 50s, doesn’t mean you have the ‘hair loss genes’.

As a general rule, however, the risk of getting bald increases with the number of family members in your familiy with hair loss – including both family members on your mother’s and your father’s side of the family.

How is hair loss diagnosed?

Diagnosing hair loss usually isn’t that hard – meaning that you don’t have to have a medical degree to see if someone is bald or not…

Examining the scalp in a person who suffers from hair loss, shows hairs with different thickness and decreased number of hairs pr. cm2.

As mentioned earlier, this hair loss sign in called miniaturization, and is caused by the negative influence of the male sex hormone, DHT.

How to treat hair loss in men?

While there are many products on the marked which state to treat hair loss, the sad truth is that only few of these products are scientifically proven to increase hair regrowth…

Finasteride works excellent for treating androgenic hair loss in men aged 20-50 years who suffer from:

  • Mild to moderate hair loss

In men with extensive hair loss (i.e. Norwood V-VII), hair loss is usually better treated by hair transplantation and post-operative oral finasteride to stop further hair loss after surgery.

Diffuse thinning in young men, however, can also be treated by Finasteride with excellent results.
